Please plan to arrive 10 to 15 minutes prior to your appointment time. This allows time for initial chart set up: copy your driver’s license, insurance cards, make sure that records are in order, fill out all necessary paperwork. Our physicians and staff make every attempt to be prompt and on time because we respect your time and ours! Your wait time will be minimal. In the unexpected event that you have to cancel your appointment, a 48 hour notice is required and greatly appreciated or there is a $25 rescheduling fee. We never “double book” appointments.

 

In consideration of all patients and staff, we ask that you refrain from using any and all perfumes, colognes, and lotions before and while visiting the office. The day of your appointment, Please bring all medications you are currently taking. This includes Supplements and Vitamins. Please bring the actual bottle with you.
